Lambeth
The Focus Project is a resettlement support service for men with a mental health illness, history of offending and complex needs. The service is based across 5 sites in Lambeth and offers high, low and medium support depending on service user need. They support clients to move on to independence or lower-supported accommodation for up to 2 years.
They work with clients to develop their own recovery plan based around the SLaM and Lambeth support recovery model and Recovery Star. They enable this through a range of support programmes and work collaboratively with other organisations within Lambeth to help service users to reintegrate into the community. They reduce and minimise risks of re-offending and keep the community safe. Their early intervention in the management of mental health relapses reduces the cause of long term hospitalisation.
